The PHQ-9 and GAD-7 are globally applied assessments for the monitoring of Depression and Anxiety symptoms. Depression Log allows you to monitor your depression and anxiety symptoms using the PHQ-9 and GAD-7 questionnaires.

It can either be used by an individual for self monitoring or it can be upgraded to allow for mental health professionals to monitor the progress of their clients.
